On 7 August 2022, a Model 1124 business jet departed Goiânia, Brazil, to conduct a pressurisation test. During cruise at FL340, the crew identified a pressurisation system malfunction and elected to return to departure aerodrome. Following a visual approach to runway 32, the aircraft landed but, during the landing roll, it veered off the runway to the right, travelled across open ground, and collided with an airport perimeter wall
The investigation identified that the accident resulted from a breakdown in approach stability, energy management, and landing decision-making. Flight data showed significant variations in the rate of descent below 500 ft, indicating the aircraft was not stabilised during the final approach phase. Standard guidance requires that, under visual conditions, the aircraft should be stabilised by 500 ft. If not achieved, a go-around should be initiated.
Video evidence showed the aircraft was still travelling at approximately 80 knots in the final third of the landing distance.
Continuing the approach was assessed as an inadequate evaluation of landing parameters and associated risks. The investigation concluded that limited experience on type may have influenced judgement.
Weather conditions were CAVOK, weight and balance were within limits. No system faults affecting controllability were identified.
The report states that no corrective or preventative safety actions had been implemented by the operator following the accident.
ANAC was recommended to work with the operator to ensure improvements in several critical areas. Crew qualification and verification of experience, Adherence to stabilised approach criteria. Improved operational risk management. Ensuring flight recorders (CVR/FDR) are fully functional and usable. Proper maintenance of operational documentation and compliance with regulations.
